To mail in your submission via U.S. Postal Service, simply print and fill out this form page (be sure to review the Entry Guidelines). Attach your Semi-Homemade recipes and/or ideas, along with a personal photo and/or video (accompanied by signed Home Video Release Forms if submitting home video tape).
